CANCELLATION POLICY:

As a courtesy to other patients, please call to cancel appointments by 12:00PM (noon) on the day prior to your appointment. If your appointment is on a Monday, please call to cancel by noon on the prior Friday. Thank you!

In an effort to comply with the latest HIPAA requirements we cannot email confidential health information related to the diagnosis, treatment, or financial status of any patient. If you have any questions related to your personal information please call our office and we would be happy to assist you. 

healthcare RECORDS:

In keeping with current state and federal regulations governing the retention and destruction of patient health records, notice is hereby given of our intent to destroy, by shredding, the records of those individuals who last received treatment seven years prior to the current year and who are not currently active patients. Destruction of medical records is scheduled to begin after January 1st of the prior seven year period. For example, as of January 1, 2022 all records from 2014 and prior will be scheduled for destruction. You may contact us at 303-772-3982 if you wish to pick up your medical records before destruction is set to begin.
